Nomination Checklist

Please read through the entire nomination form before beginning to ensure you have all the required information. Please keep the following in mind:

✔ The organization or individual nominated does not need to be a Circular Arizona member (but only members can nominate).

✔ You are more than welcome to nominate yourself or your organization!

✔ You must provide the nominee's contact information so we can contact them if they are selected as an award winner.

✔ While not required, you are encouraged to attach photos or other supporting documentation to your nomination.
